In-Network Insurance
I am an in-network provider with the following insurance plans:
Blue Cross Blue Shield
Aetna
First Choice Health
Medicaid
Coming Fall 2026: Cigna / Allegiance, Mountain Health Co-Op
Insurance coverage varies by plan. Even when a service is covered by insurance, you may be responsible for a copayment, coinsurance, deductible, or other out-of-pocket costs. Insurance plans may also have limitations on covered services or the number of visits allowed.
Before your initial appointment, I recommend calling the member services number on the back of your insurance card to verify your mental health benefits and confirm coverage for psychiatric services.
Out-of-Network Insurance
If I am not an in-network provider with your insurance plan, payment is due in full at the time of service. I will provide you with a detailed receipt or superbill that you may submit to your insurance company for potential reimbursement according to your plan's out-of-network benefits.
I do not submit claims to out-of-network insurance companies. Any reimbursement is determined by your insurance plan, and you are responsible for submitting the superbill and following up with your insurance company.
Coaching Services
Coaching is a separate, self-pay service and is not covered by health insurance. Coaching clients are responsible for payment in full at the time of each session according to the fees and payment policies outlined in the Coaching Payment Agreement.
